The US's NIH website actually used to specifically recommend that for post op trans women every five years or so. Obviously trans women can't specifically get cervical cancer. But the same test, a pap smear, tests for other vaginal cancers that post-op trans women and nonbinary folks can get. Anyone with a vagina needs a pap smear, regardless of gender assigned at birth or transitioned gender.

doctors are absolutely scary. my advice is just to tell them exactly what they are expecting to hear. before i found a good gender doc i just ran them down the checklist for gender dysphoria essentially, even though that didn't perfectly match my experience.

- Each of their albums is so incredibly different from the others so I think the answer to that question kind of depends on what you're interested in hearing. If you want a little bit of a bunch of styles, Omnium Gatherum is fun and opens with a 20 minute jam session that is SO good. Nonagon Infinity is also freaking fantastic and a great starting point in my opinion.

Hello gamers, I got my blood work back and don't really know what to make of it

T: ~90 ng/dL
E: ~65 pg/mL

Is this normal for 2.5 months on HRT? I was on 4mg estradiol a day for the first month and a half and then bumped up to 6mg, plus 100mg Spiro a day (unchanged throughout). I definitely missed doses here and there, but maybe only a handful throughout.

E doesnt come in ng/dL, are you sure you are reading the levels correctly? It should be pg/mL or pmol/L.

Not really surprising. There's a lot of variation from person to person it seems, so starting doses often are not enough, but serve to get an idea of what is needed.
It's pretty low. Also make sure you take your blood test as close as possible to right before the next dose (of pills or of an injection). This is the trough level and it's what we base our adjustments on. If you took the blood at any other time it will seem that the level is high, but this will be useless for making any conclusions.
If you are going to let the pill dissolve in your mouth, then spread it out over the course of a day. Don't put all your pills under your tongue/near your cheek all at once. If you need to split a pill in half to be able to take it throughout the day, then use a pillsplitter to make your life easier. It will take 30 mins to an hour to dissolve a full 2mg pill, anything left over in your mouth after that is filler material in the pill, just swallow it.
- Hmm the blood was taken around 2pm, and I take the pills at like 6:30am and 8:15pm. So my real levels might be even lower? Dang
- my PP doctors have me taking my levels mid way through the week, so i inject saturday and go in on wednesday for the blood draw; is there merit in going in on friday instead because I am getting checked again to see if monotherapy is working in a couple months.
  I don't really want to lie to them but i will if they are off about this!

Yeah this is rough. I just epilate for the most part so I don't have a to of hair to shave, and then generally only shave body hair 1-2 a week (or before special occasions) I definitely have to shave not in the shower if I want complete coverage.
9 months of practice and i still see hairs I missed on my knees every time i drive. Shaving is so frustrating. I can't see very well either and my hair is fine and light so i have to go by feel pretty much.
My wife has taught me that if you up the frequency the spots you don't miss overlap more but that's if you shave every day or so and i don't bathe frequently and i don't want to bathe just to shave!

I was heavily influenced by classic prog (like Dream Theater and Fates Warning) when I wrote this one.

I like how the story started to play out in

Adventure 2 but gameplay wise I feel it started to enter the era of "do all these cool tricks as we intended to get a perfect score" compared to adventure 1 which was just about speed. Like your score is still heavily tied to your speed but each little section if you break it down is just tight action sequences you have to nail in between the next action sequence... I miss the free form style of Adventure 1 so much

Thinking of getting back into Dead State. It's a spiritual sequel to Fallout 1 and 2 (and Tactics now that I have recently replayed that). It's a turn based squad RPG set in a zombie apocalypse. It's one of the few games that I feel properly balances combat against humans versus combat against zombies, exploring and looting, while managing a base, with speech checks and dialogue.

I finished it a few years ago but I messed up and caused some people to die and didn't get the best ending with a couple of others. I also picked a kind of boring ending where the surviving military helps you evacuate, in exchange for your character being drafted as a commander of their forces.

What I really like is you have to balance the personalities and factions in your base. You can't please everyone as different factions are diametrically opposed. But you can call in favors and persuade people to soften the blow. The other thing is you also manage your individual relationship with other survivors, but without any spoilers some survivors secretly (or openly) are real pieces of shit, so you're better off antagonizing them and bringing them into line (even killing or exiling them), instead of placating or appeasing them, because if you do the later messed up stuff can happen between them and other survivors. Likewise other survivors who seem like dicks or useless really come out of their shells and become real team players.

The combat is basically like Fallout 2. You have action points, 2 weapon slots, you balance attacking versus moving etc. You can customize your own character to be good at melee, range or a mix of the two (my favorite as there's some neat synergies that can make you a real jack of all trades MVP in a fight, but that's balanced against not being the worlds best sniper or melee fighter).

You also go from being like Season 1 walking dead helpless survivors to unstoppable killers in customized body armour and weapons, clearing rooms with homebrewed nerve gas.

Except I can't get it to play argh. Probably a way but it's annoying how some old games are basically unplayable now on modern computers

Thoughts on estradiol cypionate vs. enanthate? I'm looking to switch to injections and want one of the once weekly formulations.

Switched from the cheaper nail polish ($1-2 a bottle) to more expensive polish ($5-6 a bottle) and it's actually lasting several days with minimal need for repairs. I guess my nails aren't too long right so that also helps. I was thinking of taking them off actually so I could apply cuticle oil, but then I realized that you can just apply cuticle oil to the cuticles without getting any on the nails so now I feel silly for not oiling them over the last few days.

Also, piercings healed up enough to change out to new ones (8+ weeks). I was getting a bit worried since I started wearing some hinged loops and I was feeling some tenderness/warmth. I switched back to my original studs and I don't think I have an injection, but my piercings are easier to clean with the studs in so I am going to wear those for a couple of days and then resterilize any new earrings before changing them again.

just go for fixed needle syringes, much much lower deadspace (needle switching is generally unnecessary anyway)
You could use the airlock technique. That's what I use with my luer-slip syringes (and other people I know also use that technique). Once you've drawn your desired amount of E, withdraw it from the vial, point the needle up, pull the plunger to draw around 0,2 mL of air in, switch needles now if you want to use a different needle for injecting, and now point the needle downwards and let the air accumulate as a single bubble at the top with the plunger. It is now ready to inject. Inject the E together with the air (E first, then the air). What happens is that once all E has been injected, the air in the syringe will push all the E that's stuck in deadspace out, so the loss from deadspace is reduced.
Before you panic with the idea of injecting air, know that small amounts of air like the one mentioned are perfectly fine to be injected SQ or IM (I think you can theoretically also inject a whole mL and be fine). But absolutely do not inject any bubbles when administering substances intravenously though.
Note that you may want to adjust your dose now, as you will now be injecting more E than you did before.
- Your body can absorb a certain amount of air but the bigger problem with it is it usually hurts more for not much benefit. Technically you're giving yourself subcutaneous emphysema but it's really not a big deal if its a couple mls and you only inject once a week or whatever. The amount you "lose" in the needle is negligible, just think of how tiny the volume is with that gauge of a needle times it's length - it's essentially nothing. A 27 g needle is like a fifth of a mm for its inner diameter. The benefit of squeezing out that little bit of E vs the risk of harm and the actual pain it'll likely cause is just not worth it. But if that's your practice and it works for you and you don't mind the pain, go for it.
  You can technically inject a small amount of bubble IV cause your body will absorb a tiny embolism like that - but at work, if I do a couple bubbles, someone else does, som other people do - it adds up. I stick with a no bubble tolerance for IV and IM injections as well.
  I didn't know the air lock thing had a name but that's how I do it at work and for myself! Except I clear the syringe of air after switching needles lol.
If you can't find a syringe that fits the bill, there's one hack you could use. Let's say you're trying to inject .1 mL of E medicated oil.
First, obtain a vial of sterile oil of some sort. You want a sterile vial with an injection port that contains sterile oil inside. (Ideally, this would be the same oil as whatever is used as the carrier oil for your medication.)
When giving yourself a shot, first draw .5 mL of the plain sterile oil. Next, draw .1 mL of your E medication. You now have a syringe that has .1 mL of E oil sitting on top of .5 mL of sterile filler oil.
Inject yourself with the syringe. The E oil will be entirely injected, and all the dead space inside the syringe and needle will be filled with the dummy oil. You'll inject yourself with the full amount of E oil and a little bit of dummy oil.
I don't see any reason why this wouldn't work. Finding sterile oil vials could be tricky, but it's probably doable. Sterile vials of bacteriostatic water are a lot easier to find, but I'm not sure if that would cause problems with the differential density between the oil and the water. The oil would want to float to the top of the column (though this may not occur in the time it takes to draw and inject.) While this would be fine with drawing (oil on top, water on bottom), when you invert the syringe to inject downwards, the oil and water would want to switch places. You could try to inject yourself with the needle pointed upward, but that would be a bit of a feat of contortion. Though, if you're injecting into the top surface of your thigh, I suppose you could lay stomach side down between two chairs, with your thigh exposed in the gap in between. It would be a minor feat of contortion, but it wouldn't be too hard.
I would suggest trying to find vials of sterile oil. If you can't, sacrifice a syringe to science and experiment. Try drawing some ordinary tap water and some olive or other oil into a syringe. First draw some tap water into the syringe, then draw some olive oil. See if the system is viscous enough that the oil will stay on the needle end of the syringe even if inverted. If this works, then you should be able to do this dual layer technique even with more easily obtainable vials of bacteriostatic water. Obviously don't inject yourself with this experimental syringe.
Some dead space in your needles is inevitable. But there's nothing that says that your actual medication needs to occupy that dead space.
