Thank you for sharing. This is an important topic, and I'm glad that we are looking for feedback so we can shape the program collectively.
Thanks for posting this. Filled it out, and really had to think about how I felt for some of the answers.
The questions are pretty good. One thing that they didn't include is the spectre of people choosing MAID due to deficiencies in the health care system, rather than the medical condition itself. I had to write that one in.
