All part of the shower. You use it to adjust water flow and temperature.

In some places they have in-line water heaters like this instead of big hot water tanks.

Yeah, as the other guy said, it’s a water heater. It’s more common in the U.K. than the tap and a central supply type.
Under or over counter water heaters are also becoming more common. Not sure why…maybe efficiency.
